Please download the Claim Form and attach a good scan copy of the following documents:

1. Copy of flight itinerary
2. Certificate of assurance
3. Original medical reports from the treating doctor
4. Original bills/receipts issued by the clinic or hospital

Medical report must show the following information:

• Symptoms presented with
• Past medical history (if any)
• Vital signs at the time of examination
• Investigation/laboratory reports with X-ray/CT/MRI (if performed)
• Medications given
• Diagnosis

Please note we may require other documents, which we will advise upon evaluating your case.
