Please download the Claim Form and attach a good scan copy of the following documents:
- Copy of flight itinerary
- Certificate of assurance
- Original medical reports from the treating doctor
- Original bills/receipts issued by the clinic or hospital
Medical report must show the following information:
- Symptoms presented with
- Past medical history (if any)
- Vital signs at the time of examination
- Investigation/laboratory reports with X-ray/CT/MRI (if performed)
- Medications given
- Diagnosis
Please note we may require other documents, which we will advise upon evaluating your case.
